DHS submits plan to reform health care delivery, payment systems

DHS and the Minnesota Department of Health recently applied for a grant from the Centers for Medicare & Medicaid Services Innovation Center to reform the state's health care delivery and payment systems. The State Innovation Model grant would support a comprehensive statewide initiative to close the current gaps in health information technology, secure exchange health information, quality improvement infrastructure, and workforce capacity needed to provide team-based coordinated care.
