Annual Reporting Forms

All telephone companies that had operational or conditional authority in Minnesota in 2012 must file a 2012 Jurisdictional Annual Report, regardless of revenue amount. 

To submit your Report:

  • Download the Annual Reporting Form from the table above.
  • eFile your completed form under Docket No. 13-3
  • DEADLINE: All forms must be filed by May 1, 2013
