Online Address Change
All Board licensees must update any change of name, public, mailing or business address, or telephone numbers(s) within 30 days of any change. Licensees may make such changes via the Online Address Change service. The Board also requests that a current email address be on file.
Effective January 1, 2015 - the Board has a NEW ONLINE LICENSEE system through which you will conduct all future license renewals, address changes, etc. When accessing the system for the first time, you will need to register as a new user. You will create your licensee account by entering your full name, SSN (last 4 #'s) and date of birth and then creating an individual user name and password. This account will be used for all your future online license activity (renewal, name/address changes, etc.), so maintain a copy of your login information.
