ETCs are required to send two separate notices to their surveyed customers informing them of the verification process. Pursuant to the Commission's February 23, 2009 Order in Docket Number 08-659, the Department has developed two sample letters that carriers must use along with the customer verification form. You can download the letters and form from the column on the right. ETC's may revise the letters subject to the Department's or the Commission staff's approval. You may also download the PUC Order from Docket 08-659, and Docket 09-518 and the Commission's March 14, 2011 Order in Docket No. 10-211 to review.
The 2011 Minnesota ETC Lifeline Survey Forms are due on September 1, 2011. All companies must use the 2011 Minnesota ETC Lifeline Survey Form that is available and can be downloaded from the right-hand column. These Forms should be completed and eFiled to the Existing Docket No. 11-212.
Carriers shall randomly sample Lifeline recipients. The required sample size in for each company are set forth in Attachment 1 of the Commission's March 14, 2011 Order in Docket No. 10-211.
