All telephone companies that had authority (Including Conditional) in 2011, are required to file the 2011 Jurisdictional Annual Report regardless of the revenue amount. Please submit the completed annual report to the department via the electronic filing system https://www.edockets.state.mn.us/EFiling/home.jsp
